Before your first visit at my office, please print out and complete the following intake form. This will save our time on your first visit.

If you want me to coordinate your treatment with other health care providers, school officials or counselors, other professionals or family members, I will need your specific written permission. To give this permission please print out and complete the following form:
Consent to Release Information
If you have insurance that you would like to use to cover the cost of my services, it is important that you print out the following form and call your insurance carrier to verify your coverage before you come for your first visit.

Financial Policy
I accept insurance policies including BCBS, Cigna, United Behavioral Health (UBH – United Health Care), NC Medicaid and NC Health Choice. Most EAP and Health Savings Account plans also cover the cost of therapy. Please see the Insurance Verification Form. If you have any other questions, please feel free to discuss this with me personally.
